US Nurse English Requirements 2026: VisaScreen Scores

An internationally educated nurse does not face one United States English requirement. There are two separate decisions: the federal English requirement inside healthcare worker certification, usually VisaScreen, and any English requirement imposed by the state board that issues the nursing licence.

For a registered nurse, the federal table effective 12 May 2026 accepts six test options. IELTS Academic requires 6.5 overall and 7.0 in speaking. OET requires 360 in speaking, 340 in reading, 350 in writing, and 320 in listening. TOEIC is no longer on the current table.

VisaScreen and state licensure are separate

The word requirement hides three different gates. Keeping them separate prevents the most expensive planning mistake in this process.

HRSA's current page expressly warns that meeting its English standards does not itself authorise entry into the United States or professional practice. NCSBN's international nurse page explains the other half: each state is responsible for licensure, and variation exists among states.

This means a nurse can pass the federal English row and still need to satisfy a state board. It also means a state licence does not remove the federal certification step for an occupational visa.

Current VisaScreen English scores for nurses

HRSA published a new table effective 12 May 2026. Registered nurses sit in the column for registered nurses and other bachelor's-level healthcare workers.

The TOEFL values use the current 1 to 6 scale. Do not compare them directly with an older article that gives an overall score out of 120. Our TOEFL rescaling check explains how to identify whether an authority has converted its thresholds.

Speaking is the highest component in every current row. That is not marketing language. It is visible in the federal table across Cambridge, TOEFL, IELTS, MET, OET, and PTE. We compare that pattern in every US healthcare test ranks speaking highest.

The practical test choice is not simply the one with the lowest looking number. IELTS, OET, TOEFL, PTE, MET, and Cambridge use different scales and different task types. A nurse should compare availability, result timing, retake rules, task familiarity, and the separate state board list before paying.

The exam date decides which table applies

The change is attached to the date of the English exam, not the date a nurse happens to read the guidance.

TruMerit states that an English proficiency exam taken before 12 May 2026 is assessed under the previous requirements. The new HRSA table applies from that date.

That distinction matters most for these changes:

• TOEIC appeared on the previous federal list and is absent from the current one.
• PTE speaking moved sharply upward on the new scale requirements.
• OET now uses numerical skill scores in the current table.
• Cambridge changed from its earlier overall and speaking thresholds.
• TOEFL now appears on its new 1 to 6 score scale.

One source-reading rule is especially important here. The live TruMerit page links to the dated May 2026 HRSA table but also retains an older December 2024 PTE note lower on the page. When a dated federal table and an older note differ, use the table that applies to the exam date and confirm the result with TruMerit before ordering score delivery.

TruMerit also says it does not accept at-home versions of English proficiency exams. Booking an online product with a similar name does not make it acceptable.

NCLEX does not replace the English exam

For registered nurses, VisaScreen includes a nursing knowledge check as well as English evidence.

TruMerit's VisaScreen page says it needs proof that an RN passed either the TruMerit Nurse Qualifying Exam or NCLEX-RN. The same page separately requires passage of an accepted English proficiency examination.

The two tests answer different questions:

Passing NCLEX is therefore not an English exemption. It satisfies another part of the file.

NCSBN also confirms that all US nursing boards use NCLEX-RN for registered nurse licensure, while state-specific requirements for internationally educated applicants can include English examinations and credential evaluation. Its nurse licensure guidance tool routes applicants to the state rules that apply to them.

Who can be exempt from the English exam

There are two important exemptions, but they apply in different circumstances.

English-based professional education

A VisaScreen applicant can be exempt if both conditions below are met:

1. The entry-level professional education occurred in the United Kingdom, Australia, Canada except Quebec, New Zealand, Ireland, or the United States.
2. English was the language of instruction, including the language used in the textbooks.

The country condition alone is not enough. English instruction alone is not enough if the entry-level programme was outside the named locations.

The phrase entry-level also matters. The relevant course is the professional education that prepared the applicant to enter nursing, not simply a later degree taught in English. This resembles the evidence traps described in when a degree taught in English is not enough.

US work experience on renewal

The second exemption applies only to VisaScreen renewal. TruMerit lists three conditions:

1. At least 27 months of employment in the United States, including nine months before the renewal order.
2. Employment in the same profession for which the VisaScreen service is requested.
3. An employment summary signed and submitted by the US employer on corporate letterhead.

TruMerit's renewal material says a VisaScreen certificate is valid for five years from issue. A nurse should not treat the 27-month rule as a first-application shortcut.

State licensure can set a second English bar

There is no single national English table for nursing licensure. The federal table applies to healthcare worker certification. Each state board decides its own licensure evidence.

Virginia provides a current, concrete example. Its Board of Nursing requires an English test for certain internationally educated applicants and publishes these accepted scores:

The comparison reveals why one universal answer is unsafe.

For TOEFL, the federal registered nurse row requires 4.5 overall and 4.5 speaking. Virginia asks for 4.5 overall and 5 speaking. A result at the federal speaking minimum would therefore miss Virginia's published speaking minimum.

For TOEIC, the difference runs in the other direction. TOEIC is absent from the current federal table, while Virginia still lists it for its state licensing decision. Passing the Virginia TOEIC row would not put TOEIC back onto the federal VisaScreen list.

For MET and OET, Virginia's published scores are lower than parts of the current federal row. The applicant still needs to satisfy the stricter requirement that applies to each separate decision.

This is why the destination should be a state, not merely “the USA,” before the nurse books an exam.

A safer order for an international nurse

Use this sequence before paying for a test or document service:

1. Choose the state where you intend to practise.
2. Open that state board's page for internationally educated nurses.
3. Write down its accepted tests, score rules, exemptions, credential evaluator, and result-age conditions.
4. Compare that list with the current HRSA VisaScreen row for registered nurses.
5. Select one test and score target that satisfies both decisions where possible.
6. Confirm whether the exam date falls before or after 12 May 2026.
7. Use the exam provider's official booking and preparation material for test format.
8. Send results through the channel required by the board and certifying organisation.

Do not assume the score report can be uploaded by the applicant. Authorities often require direct verification from the test provider or credential evaluator.

How we researched this guide

We began with the 42 audience groups recorded in SpeakShark's market matrix and measured United States autocomplete demand using both generic profession phrases and named VisaScreen phrases on 27 August 2026.

The generic nurse seed returned five relevant suggestions. The named VisaScreen, IELTS, and OET nurse seeds returned fifteen. The key finding came from comparing authorities rather than copying one score table: a federal TOEFL speaking pass can still sit below Virginia's state speaking requirement, while TOEIC can be listed by Virginia and absent federally at the same time.
