Physiotherapist English Requirement UK: One Score
The HCPC sets one English standard for physiotherapists and thirteen other professions, and a higher one for a single profession. Here is what it is.

The score, and who it applies to
The HCPC publishes its minimum as a two row table, and the first row covers almost everybody.

Physiotherapy is in the first row. So are occupational therapy, radiography, paramedic practice, dietetics, podiatry, orthotics and prosthetics, biomedical and clinical science, arts therapies, hearing aid dispensing and operating department practice.
Two details in that first row do more work than the headline. The overall figure is 7.0, and the floor of 6.5 applies to every one of the four skills. A candidate with 7.5 in reading and 6.0 in speaking has not met it, however comfortable the average looks.
This is the same structure other UK regulators use with different numbers, which we have read one by one: pharmacists at straight sevens, optometrists at speaking 7 with others at 6, and dental roles all on one score.
The single profession with a higher bar
One profession out of the set faces a materially harder standard, and it is worth knowing even if it is not yours, because it tells you how the regulator thinks.
Speech and language therapists must reach 8.0 with no element below 7.5. On any scale that is a large gap from 7.0 with a 6.5 floor, and it exists because the profession's instrument is speech itself.

Academic or General, the HCPC takes both
This is the detail most often got wrong in forum answers, and it is stated plainly on the HCPC page.
The HCPC accepts IELTS Academic and IELTS General tests, and it accepts both UKVI and non UKVI versions.
That is unusually permissive. Several health regulators insist on the Academic paper, so applicants arrive assuming they have no choice. If you are eligible under HCPC rules, General is often easier to book, and its reading and writing tasks come from everyday material rather than academic text.
Two conditions apply whichever you take. The test must be sat at an official test centre and not at home, and the certificate must be no more than two years old when the HCPC receives your application.

Three providers, not one
The HCPC names three accepted providers: IELTS, TOEFL Internet Based Test, and the Occupational English Test.
Choosing deliberately is worth a paragraph of thought rather than defaulting to whichever you have heard of.
- IELTS is the most widely available and the most practised, and the one skill retake rule below applies to it.
- OET is built around healthcare communication, so its tasks are clinical rather than general. Candidates who freeze on abstract essay topics often find it more natural. The one skill retake rule
The HCPC states that it accepts a one skill retake for IELTS applicants under two conditions.
- The retake is done within six months of the original test.
- The retake results are provided within the test report, alongside the original results.
The retake results are valid for the same two year period as the original.
That is a genuine safety net for the candidate who misses one element by half a band, and it is not universal. Different authorities treat the same retake differently, which we mapped in which regulators allow a single component retake. Check the rule of the body you are applying to rather than assuming the HCPC position travels.
The exemption worth checking first
Before booking anything, check one list.
The HCPC treats a primary qualification from one of its qualifying countries as evidence that you can practise effectively in English, and in that case no further language documentation is required at all.

What to practise if you are short
Assume you need the test and are short in one skill. Where the shortfall sits changes what to do.
Short in speaking. This is the element that does not improve by reading about it, and the one where a 6.0 blocks an otherwise passing set. Short in writing or reading. Those respond to structured practice and past papers, and a language school or a book is usually better value than an app.
Short across the board. Then the honest answer is time rather than technique, and the two year certificate window means there is no advantage in sitting early and having to repeat it.

Registration and the visa are two different gates
Physiotherapists arriving in the UK meet two systems that are often described as one, and they set their language rules separately.
The regulator decides whether you may practise. That is the HCPC standard above: 7.0 with no element below 6.5, from one of three providers, at a test centre, within two years.
The immigration system decides whether you may enter and work. That runs on the Home Office's own list of approved secure English language tests and its own levels, which are set by visa route rather than by profession.
The two do not automatically agree. A certificate that satisfies one is not guaranteed to satisfy the other, because they publish separate accepted provider lists and separate thresholds. The practical consequence is to check both before booking, since one test sitting may or may not serve both purposes depending on the provider and version you choose.
That separation is the same one we found in Germany, where a university sets the programme language and a state authority sets the licence language, and in Canada, where the province rather than the country decides nursing registration. The pattern repeats across systems: whoever controls the permission sets the language rule for that permission, and nobody coordinates the list.
